Medical School Education Verification Request
Letter and response form asking a medical school (or registrar) to confirm attendance dates, degree, graduation date and any adverse academic or professional actions.
Where to verify medical education and training, a searchable directory of accredited U.S. professional schools (MD, DO, dental, podiatry, optometry, chiropractic, pharmacy, PA, nurse anesthesia and nurse-midwifery), and residency and fellowship directories. Hospital affiliation verification is on the Affiliations page. Everything here is summarized from the linked sources.
On this page: Education & training sources · Professional school directory · Residency & Fellowship Programs · Training program contacts · Request templates · Looking for hospital affiliations? See Affiliations.
Each card covers what the source verifies, who accepts it as a primary source (only where the source or an accreditor says so), what it costs, and the link. Filter by category, role or cost.
Every row comes from the accrediting body for that degree. MD programs come from the LCME Directory of Accredited MD Programs (updated October 14, 2026). DO schools and teaching locations come from AACOM's U.S. Colleges of Osteopathic Medicine directory (dated 9/3/26). Dental (DDS/DMD) programs come from CODA's predoctoral program search. Podiatry (DPM) colleges come from CPME's list of podiatric medical colleges. Optometry (OD) schools come from ASCO's member schools list (members must hold ACOE accreditation). Chiropractic (DC) programs come from CCE's accredited DCP list. Pharmacy (PharmD) programs come from ACPE's program lookup. Physician assistant (PA) programs come from ARC-PA's accredited programs list. Nurse anesthesia (CRNA) programs come from the COA list of accredited programs (July 8, 2026). Nurse-midwifery (CNM) programs come from ACME's accredited programs list. All lists other than LCME, AACOM and COA were retrieved Oct 31, 2026. MD and DO website links were checked Oct 22, 2026. For other types, the link goes to the program's site as listed by the accreditor, or to the accreditor's own entry. To verify a graduate, contact the school's registrar or student affairs office, or send our education verification request.

"Link check pending" means we couldn't load the school's site from our checker, so we don't show a link. Look the school up in its accreditor's directory instead. Schools with more than one campus or teaching location are listed once per location, as the accreditor lists them. Physician assistant rows have no city because ARC-PA doesn't publish one.
These accreditors publish searchable directories that are too large, change too often, or can't be read reliably by our tools, so we link to them instead of copying rows.
NP programs are graduate nursing tracks accredited at the school level by CCNE or ACEN. There are hundreds of schools, and the lists don't break out NP tracks, so we link to both search tools. Also confirm NP board certification (AANPCB or ANCC).
Search ACEN-accredited master's and doctoral nursing programs, including NP programs, by state and level.
Clinical, counseling and school psychology doctoral programs (PhD and PsyD), plus internships and postdoctoral residencies.
AuD programs are accredited by ASHA's CAA or by ACAE. The CAA listing returned an error during our Oct 31 check, so use ASHA EdFind.
CAPTE accredits DPT and PTA programs and lists them with status.
ACOTE accredits OTD and master's OT programs (and OTA programs).
CAA (anesthesiologist assistant) master's programs are accredited through CAAHEP.
There are thousands of programs, so we don't list them here. Use these two free official directories:
Search ACGME-accredited programs and sponsoring institutions by specialty, state or name for the current academic year. Use it to confirm a program's accreditation and its sponsoring institution.
Lists 6,000+ ACGME-accredited residency and 8,000+ fellowship programs (AMA figures). Each program page shows a Program Director and a Primary Program Contact (often the coordinator) with email and phone.
Contacts change every year, so check FREIDA or the program's own site each time rather than saving old contacts.
Where some programs ask you to send residency, fellowship and training verifications, taken from each institution’s own page on Oct 31, 2026. Contacts change, so confirm on the linked page before sending a signed release.
